To submit your Additional Required Forms, please log into My Account, then download, complete, and upload the form back to your MyAccount using the Additional Required Forms Upload Tool (in the Forms section). 

If you are experiencing an error when uploading your documents, please confirm the document's file size is no more than 4MB. You can upload as many documents as needed.
